About the Anthem SmartSense Plan:

This plan provides more comprehensive coverage before the deductible than our CoreShare plan and also offers the choice of additional benefits (dental coverage, life insurance coverage, or supplemental accident coverage) that you can add or remove (at a change in cost) to suit your ever changing needs. This way you don’t pay for coverage that you don’t actually want or need. You have the flexibility to choose your deductible and your maximum out of pocket expense. It also offers prescription drug benefits for generic and brand name medications before deductible. You have access to our extensive PPO network and like all our plans, your doctor usually fills out the paperwork and files the claims, saving you time and hassle. As always, no referrals needed, ever!

• All covered out-of-pocket expenses, except prescription drugs, apply to the expense limit
• Predictable copayments for in-network doctor visits, before your deductible
